Building Capacity for Social Determinants of Health Programming at FQHCs

Join the National Nurse-Led Care Consortium as we discuss the intersection of medical and financial health and the impact of financial literacy programming on health outcomes of patients.   This webinar will provide an overview of readiness and needs assessment of expanded SDoH programming, how financial health can provide a foundation to elevate social determinants of health (SDOH) programming  at FQHCs and improve health outcomes.  Case studies will highlight successes and challenge of supporting financial literacy programming at FQHCs. 

Attendees will (1) increase awareness of SDOH programming in the field, (2) learn about integrating financial literacy efforts in a clinical setting, and (3) have an opportunity to join a learning collaborative around SDOH programming.
